Options for Continuing Voluntary Benefits
Flexible Spending Accounts (FSA)
If you currently contribute to the Unreimbursed Health Expenses FSA, your FSA participation ends on the date of termination or on the last day of the pay period in which the Participant has contributed, whichever gives the Participant the greater period of coverage. If you leave county employment between December 22 and December 31, your participation will end on December 31, since this is the last day of the FSA plan year. Your Benny Card will be disabled when FSA participation ends. You will be offered COBRA continuation coverage by PacificSource Administrators if you leave employment prior to the end of the plan year. If you currently contribute to the Dependent Care Expenses FSA, reimbursements of Dependent Care expenses include the period of coverage and/or following termination - that is, through the remainder of the Plan Year. Aflac Insurance
If you purchase Aflac insurance policies, you will be covered under your current Aflac policies through the end of the month in which you leave employment. The Aflac premiums for your final month of employment will be taken from your final paycheck. Human Resources will notify Aflac of your departure. If you do not want to continue your Aflac coverage after leaving employment, there is no action is required on your part. However, if you would like to consider continuing your Aflac policies after leaving employment, contact Deschutes County Aflac representative Becky Swanson at 541-531-5759 or via e-mail at rebecca_swanson@us.aflac.com to discuss your options and to make future premium payment arrangements.
Legal Shield
If you purchase Legal Shield pre-paid legal services, you will be covered under this coverage through the end of the month in which you leave employment. The Legal Shield premium for your final month of employment will be taken from your final paycheck. Human Resources will notify Legal Shield of your departure. If you are interested in continuing your pre-paid legal services through Legal Shield, you may complete a Payment Option form. E-mail the completed form within 30 days to Memberservices@legalshield.com using the same e-mail address that is on file for your membership.
