Community First Choice State Plan (K-Plan)
Community First Choice State Plan (K-Plan) allows Oregon to provide home and community-based services and supports while receiving a 6% increase in federal medical assistance funds from the federal government for those services.
The services provided benefit Oregonians who want to stay in their home and remain independent, healthy, and safe. At the same time, it saves both the state and federal government money because we are providing more extensive supports and services in a less restrictive environment meaning in lieu of more expensive institutional-type care.
An individual must be receiving services through Deschutes County Intellectual/Developmental Disability Program and be on the Oregon Health Plan in order to receive this service. Individuals who are not currently on the Oregon Health Plan may be eligible for K-Plan services based on Presumptive Medical Eligibility; i.e. based on individual's income and resources only.

•Emergency Response Systems
•Environmental Modifications
