Babies First! is a voluntary program that provides nurse visits for pregnant women and families with babies and young children up to age 5. Babies First! helps make sure families and babies have the support and information they need to be healthy.
Having a baby is an exciting time, but it can raise a lot of questions. Knowing you have extra support will boost your confidence. Nurses visit biweekly or monthly.
Babies First! nurses partner with families to provide:
CaCoon (CAreCOOrdinationN) is a statewide, public health nurse visiting program in partnership with OHSU. CaCoon nurses help families coordinate care for their children and youth with special health needs.
Nurses visit families as needed at home and in the community such as at doctor’s offices, at specialty appointments, IEP meetings, and etc.

The Family Connects Oregon program supports parents by linking nurses, community resources, and families. Our mission is to connect you with resources that nurture your whole family and support your child. It’s about planting seeds for the future. Together, we’re growing healthy babies!
Family Connect Nurses partner with families to provide:
The Perinatal Care Coordination model is designed to increase access and enrollment into available perinatal resources during the prenatal and postpartum period for individuals and their families. We currently have staff embedded in local OBGYN clinics providing onsite support as well as onsite at our local public health departments.
