Our South County Hub provides comprehensive outpatient services to Adults, children, adolescents, and families. Our multidisciplinary team offers a full range of services to residents of Southern Deschutes County and Northern Klamath County. We utilize an integrated approach that treats co-occurring mental health and substance use disorders, partnering with medical providers to support the whole health needs of the people we serve. We work collaboratively with our clients to develop a treatment plan to support their goals.
What services does SoCo offer?
- A comprehensive screening and assessment to learn more about challenges
- Individual, family and group counseling for all ages and backgrounds
- Case Management - for care coordination and skills training
- Co-occurring substance use treatment to support mental health and substance use recovery
- Peer Support Services – for mentoring, education, and assistance in navigating the treatment process
- Medication providers who can provide medication options to support mental health and recovery
- Nurse access to assist with medication support and coordination with Primary Care Providers
- Vocational and employment support
- Bilingual service providers and access to language interpretation services
- Parent / Child Interaction Services (PCIT)
- Care Coordination and Referral services for continued care and building community connections
Eligibility
Individuals with mental health and/or substance abuse diagnoses who are willing to voluntarily participate in services. Diagnoses determined by the Access Team
